Music Warehouse Forms

Participation Requirements

Age: Grades 3 through college (must have completed the second grade)

Cost:

  • $15.00 for Youth Chorus (grades 3 through 8)
  • $25.00 for Senior Chorus (grades 9 through college)
  • ($50.00 maximum per family)

Parents: Must attend Mandatory Meeting (June 6, 2010 6:00 PM at the former Steve and Barry's location at the Miami Valley Center Mall

All youth must audition, Youth and Senior Chorus (check calendar for dates and times)

Youth Chorus will audition in front of a 3-judge panel. Must match pitch.

Senior Chorus members will have costume measurements at the audition. If you are interested in a solo, please pick up a Solo Packet at the Piqua Area Chamber of Commerce starting June 1st.

Everyone MUST bring a completed "Forms Packet" to the audition!!!

Forms Packet

All youth who wish to participate in Music Warehouse must obtain a "Forms Packet" and submit completed forms when they audition.

Forms Packets can be downloaded or printed from the links below or obtained in person at the Piqua Area Chamber of Commerce starting June 1st. The Piqua Chamber is located at 326 N. Main St. in Piqua.

  • Sunday, Singing
  • Tuesday & Thursday, Dancing
  • Youth Chorus 6-7 PM
  • Senior Chorus 7-9 PM
  • Rehearsals start: Date to be announced
